OPCAB

National Research Heart Surgery Center (NRHSC) regularly holds seminars, courses and trainings.  We invite heart surgeons, cardiac anesthesiologists, surgery nurses to "OPCAB Class" (Myocardial revasculization on a beating heart) for education on the basis of multidisciplinary simulation center NRHSC.

OPCAB is a task for a multidisciplinary team. The aim of the Class is to support colleagues who have little experience in the operations of this category. The theoretical part of the Class discloses all aspects of patient management from the time of admission and to discharge, including recommendations for further treatment. The practical part of the covers and helps practice the following skills up to every little detail:

• surgical tutorial: risk assessment of surgical treatment; techniques of operating the tools, forming vascular anastomosis, manipulations during the main phase of the operation, work with equipment;

• anesthetic: surgery and anesthesia risk assessment; catheterization of arteries and veins for invasive monitoring, installation of Swan-Ganz catheter and measurement of hemodynamic parameters, intraoperative transesophageal echocardiography control method, nuances of the tutorial during the different stages of the operation;       
  

• tutorial for surgery nurse;   
  
• recommendations regarding patient management post OPCAB after discharge.         
  

The author of the Class, professor Paul Sergeant, is an effective member of various cardiac surgery communities: (Scientific Association Memberships, American Association for Thoracic Surgery (AATS), Asian Society for Cardiovascular and Thoracic Surgery (ASCVTS), Belgian Association For Cardio-Thoracic Surgery (BACTS), Cardiothoracic Surgery Network (CTSNet), European Association for Cardio-Thoracic Surgery (EACTS), The Society of Thoracic Surgery (STS)).

Prof Pya and Dr Lesbekov visited several years ago Prof Sergeant in Leuven Belgium to study a new approach in coronary surgery. He has been invited two years ago to organise similar courses in Astana as in Belgium. Already twice has Prof Sergeant joined the scholars and participated in such an Astana training. Prof Sergeant stated on June 22-23 that the surgery he could observe, namely full arterial reconstruction, no-touch aorta and in OPCAB mode was only practiced by the best of the best, probably in less than 1 % of the best centers of the world.

It is with great pleasure that we announce a next step in these trainings and a first in mobile encrypted learning. Through the collaboration of some many colleagues we hereby announce the availability (without cost) of an encrypted small personnalised online course (spoc), describing this approach of OPCAB surgery.

This course describes in detail all the aspects to allow a safe surgical approach for coronary surgery without the extracorporeal circulation. It starts at the moment the patient is evaluated for a coronary angiogram and finishes when he is ready to leave the hospital.

The course (CABG OPCAB by Meplis) can be downloaded from the android and apple stores on a smartphone or on a desktop. You will have to create your own username and password. If you are not so familiar with English, then go under settings (3 bars on the left) and change to Russian.
