The list of required documents for participants

Doctors must provide to the Department of education the following documents:

1. A copy of identity card;
2. Copy of the diploma;
3. Copy of certificate of completion of internship;
4. A copy of the certificate of the expert/specialist;
5. A copy of the certificate of professional development (last 5 years);
6. Traveling certificate;
7. Direction to study (a copy of the order);
8. The contract in 3 copies.
9. Health record book (Pluronic, ELISA deployed, HIV, HBV vaccination, ADSM)
10. Shoes
